Three years ago, a Huntly teenager went into labour for what should have been a straightforward birth.

But, hours later she and her baby were dead. Earlier this year, a coronial inquiry found the lead maternity carer was, amongst other things, inadequately trained and lacking in experience.

An inquiry by the New Zealand Midwifery Council found the midwife, and two other midwives from the same practice, incompetent.

The lead maternity carer, who has name suppression, left the country just a month after the deaths.  She left with a Certificate of Good Practice.
